Acadian Health, a leading provider of hospital-at-home services, is proud to announce its expansion into Wall Township, New Jersey. This growth is in partnership with Medically Home, supporting its model for hospital-at-home care.

Acadian Health’s hospital-at-home program provides patients with the same level of clinical care they would receive in a hospital, but in the comfort of their own homes. The program is integrated with hospitals in each region, and has shown improved patient outcomes, reduced costs, and increased patient satisfaction. Acadian Health works with patients identified by the hospital and covers a service area that fits the hospital’s goals for their communities.

Acadian Health has provided hospital-at-home services in five states since December 2023, with over 35,000 patient visits cumulatively. So far in 2025, Acadian Health has had 2,242 program admissions.

“Bringing hospital-level care into the homes of New Jersey residents is one of the ways we are transforming healthcare delivery, ensuring patients receive compassionate, quality treatment within the comfort of their own surroundings,” said Acadian Health Senior Director Jeremy Coombs.

Acadian Health’s expansion is a response to the growing need for innovative healthcare delivery models. Medically Home’s hospital-at-home care, supported by Acadian Health, directly assists hospitals and health systems in providing acute, hospital-level care to patients at home. Medically Home has assembled a complex, dynamic network that makes up a virtual healthcare infrastructure in the areas where it operates, delivering care to thousands of patients around the country.
